Who we Are



Health Opportunities Consortium (HOC) is a trusted provider of health related social support services focused on advancing equity and improving outcomes for Medicaid beneficiaries. We bring over five years of experience delivering high quality, non medical services under the North Carolina Medicaid 1115 Waiver.

Our work is rooted in addressing Social Determinants of Health (SDOH) the conditions in which people live, work, and age to create meaningful improvements in overall health and well-being. We partner with Medicaid programs, health systems, community based organizations, and local agencies to bridge critical service gaps and build healthier, more resilient communities.

Yes, we offer 29 specialized programs that are under the Healthy opportunities Pilot and Human services specicifically tailored to meet client  needs and workforce development needs

Are telehealth appointments available?

Yes, we offer telehealth appointments for certain services to provide convenient and accessible care for our clients

We have implemented strict safety protocols to ensure a clean and safe environment for all clients and staff members.  We are HIPAA compliant and clients in our care have the highest stardard of care possible.
