Who we Are
Health Opportunities Consortium (HOC) is a trusted provider of health related social support services focused on advancing equity and improving outcomes for Medicaid beneficiaries. We bring over five years of experience delivering high quality, non medical services under the North Carolina Medicaid 1115 Waiver. Our work is rooted in addressing Social Determinants of Health (SDOH) the conditions in which people live, work, and age to create meaningful improvements in overall health and well-being. We partner with Medicaid programs, health systems, community based organizations, and local agencies to bridge critical service gaps and build healthier, more resilient communities.
What do we do?
Our Core Service Areas Providing Enhanced Health Related Social Needs Services. Housing Supports: Assistance with short-term housing, eviction prevention, and housing navigation. Nutrition Services: Food access programs including medically tailored food boxes, fruit and vegetable prescription programs, and nutrition education. Transportation Assistance: Support for non-emergency medical and social services transportation. Interpersonal Safety & Wellness: Services addressing domestic violence, mental health referrals, and safe environments. Health-Related Case Management: Person-centered planning, service coordination, and care transitions.
